We've collected the mostly commonly used forms used by our current graduate students below. Other forms used by McMaster graduate students are available from the School of Graduate Studies
MS Word template. The CV information will be used by the Department as part of the nomination of graduate students for external and internal scholarships. Students should submit an updated printed copy to the Department as appropriate and at least once a year, in September.
PDF format. Sections of the report must be completed by students prior to a Supervisory Committee Meeting.
The two Transfer forms are to be completed by the Examining Committee.
The Defence Report Form is to be completed by the Examining Committe for the final
submission of a MSc thesis to the School of Graduate Studies.
This form to be completed by the Examining Committee at the end of the oral defence.
This form will be provided directly to the Defence Chair by the School of Graduate Studies.
This form is to be completed for the final submission of a PhD thesis to the School of Graduate Studies.
This form is to be completed for transfer credit, transfer from one program to another, auditing a course, change in course designation, designating a course outside one’s own program as a required course.
This extension form is to be completed for the following; time to completion including final defence, comprehensive examination, visiting scholars, course requirements (eg INC until a certain date)
Complete this form when a student requires a special change to their graduate studies not covered by the above two forms.
PDF format. Form must be completed for absences from the University of more than four weeks.
To be completed when leaving Chemistry or Chemical Biology graduate programs.
